The cost of treatment depends on the severity of the patient’s problem. You will be able to discuss fees and payment options before treatment begins, including a no down payment option and interest free financing. We also accept assignment from most insurance plans and will file the necessary papers to your primary insurance company. The following best describes our payment options:

- No down payment required.
- Low monthly payments.
- Information and credit approval is available by calling 1-800-510-5638 or online at www.chasehealthadvance.com.
Office Payment Plan | Back to top
- Interest-free financing available.
- Down payment required – amount will depend on total treatment fee.
- Monthly payment plan arranged to pay off remaining balance.
- Payments can be made by automatic draft from checking or savings accounts, or by credit card.
- Courtesy fee reduction available for additional family members who begin treatment.
Payment in Full | Back to top
- Due to savings in administrative costs, we will apply a 3% reduction in the comprehensive treatment fees if you pay in full by check or cash.
- Fee reduction does not apply to Healthcare Financing or credit card payments.
Services We Provide for Insurance Plans and Flexible Spending Accounts | Back to top
We will submit claims to your primary insurance provider. Although we will file with your primary insurance provider, you are ultimately responsible for any balance not paid by your insurance company.

It is very important that you understand the benefits your policy provides and that you provide us with accurate and complete policy information. Contact your benefits coordinator at work to answer any questions you may have. The more you know about your policy the more we can assist you with filing your claims.
Please remember that our primary financial relationship is with our patients and their families and not with the insurance company. Therefore, we will expect the responsible party to pay any outstanding balance not covered by insurance. |
